Investigations on Connective Tissue in Carcinomas of the Skin

Abstract

Histological and histochemical investigations relating connective tissue have been carried out on 300 biopsies of squamous cell carcinomas, basal cell epitheliomas and precancerous tumours of the skin. The connective tissue of the investigated tumours displayed changes on elastic fibres, collagen and the ground substance.

The clastic fibres are showing destruction but no formation of new fibres could be seen.

The most extensive changes are found on mature collagen in basal cell epitheliomas which demonstrate on one side destruction and on the other intensive formation of young mesodermal cells with production of new collagen fibres, which are in close contact with the cells of the tumour with increased numbers of mast cells around them.

Reticulum fibres seem not to be destroyed in basal cell epithelio mas.

In the ground substance from the acid mucopolysaccharides increased is hyaluronic acid and in some types of basal cell epitheliomas also chondroitin sulphate B could be demonstrated.

In adenoid basal cell epithelioma also epithelial type of sialomucin could be found which is considered to be encountered in metastatic adenocarcinomas in the skin.

It is supposed that the pathological differences between squamous cell carcinoma and basal cell epithelioma are not related so much to the histogenetic factors involved but rather as the sequel of different relationships to the connective tissue.

The PAS positive diastase removable material could be found more oi' less in both squamous cell carcinomas and basal cell epitheliomas. Consequently, this reaction appears not be reliable for their differentiation.

There are distinctive differences in location of the Hale positive material between the squamous cell carcinomas and basal cell epitheliomas.

In basal cell epithelioma the Hale positive material is formed in the stroma of the tumour and in squamous cell carcinomas predominantly in the intercellular spaces of the tumour cells forming a fine blue stained net. Similar Hale positive net could be observed in the hyperplastic epidermis in precanceus conditions of the skin.
