Importance of the Leukoplakia on the Vaginal Portion of the Uterus in Forming of the Cervical Cancer

The author reports the results of his investigations of importance of the leucoplakia on the vaginal portion of the uterus in the ethology of cancer. Up to the present the investigations showed that the leucoplakia could be one of the characteristics of being atypic process on the vaginal portion, but not always. The leucoplakia is not due to be either always together with cancer or to be one of the matrix bodies where-from cancer develops. Only in 8—10% cases, under the picture of leucoplakia, one can expect changes of the epithelium of so called “cancer in situ” or of one true, invasive cancer.
The investigations of leucoplakia on the vaginal portion were made by exfoliative cytology after Papanicolaou, colposcopy, colpomicroscopy, colposcopical portiogreametry (method of measuring changes on the vaginal portion) and histology.
The common leucoplakia frequency by colposcopical examination was 2—3% by before gynaecological uncontrolled material.
The leucoplakias make 48,3% of all findings of facultative indicators of atypical epithelium on the vaginal portion found by colposcopical examinations (mosaic-like area, leucoplakia and ground leucoplakia and their variations). The average size of leucoplakias area was about 66 m2 and middle size of the diagonal was 12 X 9 mm. From 74 examined leucoplakias, 21 were multiplied leucoplakias, and by 8 cases were found leucoplakial changes in the lower part of the cervical canal.
The colpomicroscopical examinations gave the typical picture of homogenic, white areas, without details.
The cytological examinations gave the following results: I group: 14 cases, II group: 35 cases, III group: 7 cases and IV group: 1 case, thus 49 negative, 7 doubtful and 1 positive case. The histological examinations found 2 cases (2,7%) of squamous cell cancer. The author proposes above all to remove under colposcope the existing leucoplakial keratosed surface and only then to take direct, purposive smear, in order to avoid getting “untrue negative” cytological results.
